Request to change your benefit plan

- Go to Account and select Change benefit plan.
- Describe your request.
- Confirm your contact information.
- Choose your preferred time of day for us to call you, then submit your request.
You'll receive an email confirming your request was received.

Once you submit a request, a personal benefit consultant will call you within 2 business days.
