Request to cancel your benefit plan
We know things can change, and your current plan may no longer be the best fit for you. Whether it’s due to a life event, a change in your needs or something else entirely, our expert personal benefit consultants can guide you through the process.

Sign in to your member account.
If you do not have one, you need to register for a member account.
- Go to Account and select Cancel benefit plan to submit your request.
- Review the options. If you choose to call us, you can exit the page. If you choose to have a personal benefits consultant, contact you, go to step 4.
- Select Go to the form.
- Review the important notice. Select Start the form to continue.
- Once you have completed the form, go to Continue.
- Confirm your contact information.

Choose your preferred time of day for us to call you, then submit your request.
Submitting your request will not automatically cancel your plan.

When you choose to have a personal benefit consultant contact you, they’ll call you within 2 business days to talk about your request. If they’re unable to reach you, your plan will remain active. Your premiums will be withdrawn from your account as usual.
