View your dental predeterminations

A predetermination occurs before any dental treatment takes place. It's an estimate of how much a proposed dental treatment will cost. It explains how much your plan pays and how much you may have to pay out of pocket.

How it works


Request a predetermination

Ask your dental provider to submit a predetermination to us before any treatment takes place. This way you can avoid surprise out-of-pocket expenses and make an informed decision.

How long it takes us to review

Predeterminations submitted to us electronically are typically processed within the next business day. It may take longer if we need more information. Once we have everything we need, it may take up to 10 to 15 business days to complete the review. If our dental consultants also need to review the predetermination, we may need more time.

View your predetermination

Predeterminations that are $800 or higher will be available to view in member site. If it's under $800, your dental provider can walk you through the details.

Proceeding with the treatment

If you decide to go ahead with the treatment, let your dental provider know. Once the claim is submitted, you'll be reimbursed up to your plan maximum. For orthodontic treatments, you'll only be reimbursed if we have an approved treatment plan on file.

How to view dental predeterminations in your account

  1. Sign in to your member account.

    If you do not have one, you need to register for a member account.

  2. Go to Claims and select Dental claims.
  3. Select the Treatment plans tab.
  4. Click on the verification number. This will open a PDF letter outlining the details of your dental treatment plan.
