Submit a claim online

Our website for plan members makes submitting claims easy.

Download our Alberta Blue Cross® member app or use our website to submit your claims. When we've assessed your claim, you'll be able to view your statement online anytime.

Visit the Submit a claim area of our web site or mobile app and begin by selecting the benefit category that matches your claim. Follow the steps to submit your claims and receive reimbursement faster than ever. Depending on your benefit plan, you will be able to submit the following types of claims: drugs, health, vision, dental and spending account.

Submit a claim by mail

We made mailing in claim forms easy

Some expenses cannot be claimed online so you may have to use a paper claim form. Sign in to our member web site, complete a pre-populated claim form, print it off and mail it to us with your receipts. Once it’s processed, you’ll be able to view your statement online, and if you’ve signed up for direct deposit, you’ll receive your reimbursement within a few days.

Avoid paying out out-of-pocket by finding a provider who direct-bills

Over 90 per cent of all claims submitted to Alberta Blue Cross, including claims from pharmacies, dental offices, ambulances, vision care providers, chiropractors and physiotherapists, are sent directly to Alberta Blue Cross and adjudicated electronically in just seconds at the time of service.

Almost all Alberta pharmacies and dental offices offer direct billing so they're not included in the listing

Use the direct-bill provider look-up to find the address and phone number of a provider near you. The list also gives you directions and Google Street View so you’ll know exactly where you’re going. The following types of providers are included in the list.


How funds are collected for an at-fault claim.

In the event of a benefit claim resulting from an accident due to the fault of another party, subrogation refers to the process by which your benefit provider collects money from the party at fault (or their provider) in order to recover funds that you or your provider have already paid.

When a Alberta Blue Cross® member initiates litigation to recover out-of-pocket expenses that were incurred due to an injury caused by another party (such as a slip and fall accident), Alberta Blue Cross® will be contacted by the lawyer’s office (if their client has or has had Alberta Blue Cross® coverage) to request benefits paid history report from the incurred date of injury to request date. The lawyer’s office also will inquire as to whether Alberta Blue Cross® has a subrogated claim in the matter.

We ask members to fill out the consent form which best represents their subrogation claim. If a member has more than one plan with Alberta Blue Cross®, please ensure they list all their coverage ID numbers on the consent form to avoid delays in processing.

For the HIA Section 34 consent form

  • Click on the box in the “Type of consent” section, then click on the link (underlined HIA Section 34), and that will bring you to the form.

For the “City of Edmonton (form #31010)” and “Other (form #30662)” consent forms

  • Click on the box in the “Type of consent” section for the consent that you required.
  • Scroll down to the bottom of the subrogation access form and the correct consent form will be on the next page.
